Welcome to a series of three Continuing Medical Education (CME) online courses, Medical Homes for Individuals with Special Needs. These online courses are sponsored by the Health Services for Children with Special Needs, Inc. (HSCSN).
These courses present unique characteristics of patient-centered medical homes for children with special needs; the rationale and steps for integrating behavioral health screening in pediatric practice; initiatives that can increase the success of screening, provider education, quality improvement, and mental health consultation; and components of transition of care for young adults with special health care needs.
