This portal offers a series of Continuing Medical Education (CME/CEU) online video courses: (I) Health Care Transition for Adolescents and Young Adults (1 course), (II) A Fresh Look at Autism Spectrum Disorder (3 courses), and (III) Creating a Medical Home for Individuals with Special Needs (3 courses). These oncline CME courses are free and available to a DC and national audience.

I. Health Care Transition for Adolescents and Young Adults This course offers a brief review of clinical recommendations on transition and the tested quality improvement model -- the Six Core Elements of Health Care Transition.™

II. A Fresh Look at Autism Spectrum Disorder These courses present common co-morbid conditions, current DSM-V criteria and early differential diagnosis of ASD, and indicators for youth of ABA treatment. |

III. Medical Homes for Individuals with Special Needs These courses demonstrate how to create a medical home for children with special needs, how to coordinate mental health screening and services, and how to transition to adult care. |
To receive credits and certificate, participants must score 70% or above on the online post-tests in order to apply for credits.
